New York, NY (June 29, 2012) -- Under-performance of small bowel biopsy during endoscopy may be a major reason that celiac disease remains underdiagnosed in the United States, according to a new study published online recently in Gastrointestinal Endoscopy. Investigators at the Celiac Disease Center at Columbia University Medical Center (CUMC) found that the rate of small bowel biopsy is low in this country.

"The vast majority of people with celiac disease in the United States remain undiagnosed," said lead author Benjamin Lebwohl, MD, MS, assistant professor of clinical medicine in the Division of Digestive and Liver Diseases, and a gastroenterologist and epidemiologist at the Celiac Disease Center, CUMC. "This stands in contrast to countries in Western Europe and Scandinavia, where patients with celiac disease are much more likely to be diagnosed. Are patients with celiac disease in the United States not seeking medical attention? Actually, this study shows that some of these undiagnosed patients may be coming to see a gastroenterologist and still are not getting the biopsy they need for a diagnosis."

Dr. Lebwohl and colleagues analyzed the Clinical Outcomes Research Initiative National Endoscopy Database (CORI-NED), a multicenter database that represents a broad section of endoscopy centers throughout the country, including community-based as well as academic settings. They identified all patients who underwent upper gastrointestinal endoscopy during the years 2004-2009 for one of the following reasons: Weight loss, iron deficiency, anemia, or diarrhea. "Any one of these symptoms could be compatible with celiac disease. We know that the blood tests for making this diagnosis are not perfect, and so we argue that most patients having an endoscopy for one of these reasons should have a biopsy of the small intestine to test for celiac disease."

In fact, physicians did a small bowel biopsy in only 43 percent of the more than 13,000 patients having an endoscopy during this 6 year period.

"We thought that biopsy rates would be higher, especially considering the increased public awareness of celiac disease," said Dr. Lebwohl. Although the rates increased somewhat over time, in the most recent year of the study (2009) the biopsy rate remained only 51 percent. "As a result, it remains possible that a patient with celiac disease may seek health care, see a gastroenterologist, and have an endoscopy, and yet may still remain undiagnosed."

These low biopsy rates were not uniform: men were less likely to have a biopsy as compared to women. This may provide a clue to understanding a long-observed curiosity: the lopsided gender ratio among patients with celiac disease.

"Men are just as likely as women to develop celiac disease, but women are much more likely to be diagnosed," said Peter H.R. Green, MD, professor of clinical medicine in the Division of Digestive and Liver Diseases, director of CUMC's Celiac Disease Center, and senior author of the study.

"Perhaps men aren't being diagnosed because their doctors aren't biopsying them, even when symptoms are compatible with celiac disease. This becomes a self-fulfilling prophecy: men aren't being biopsied, diagnosis rates in men remain low, and so doctors erroneously think that celiac disease is a 'woman's disease,'" said Dr. Green.

Biopsy rates were also lower among older patients, African-Americans, and Hispanics. Dr. Green added, "We know that celiac disease can occur at any age, and is not limited to Caucasians; but here again this celiac disease stereotype of the young Caucasian is reflected in the biopsy practices of gastroenterologists nationwide."

The authors are planning follow-up studies that aim to educate physicians regarding the benefits of performing small bowel biopsy.

    Warriors coach Jackson reveals extortion plot against him

    OAKLAND, Calif. (AP) -- Golden State Warriors coach Mark Jackson said Thursday that he and his family were the targets of an extortion attempt related to an extramarital affair he had six years ago.

    The Smoking Gun reported that a 28-year-old former stripper, Alexis Adams, possessed nude photographs of Jackson and wanted money to keep the photos from becoming public.

    Adams and an alleged co-conspirator, 40-year-old Marcus Shaw, were named in felony criminal complaints for their alleged roles in an extortion scheme, according to online records filed with the U.S. District Court in Oakland. The documents don't identify the victim but The Smoking Gun said "V1," as the victim is referred to in documents, is Jackson.

    Adams was released on $25,000 bail but Shaw remained behind bars because he has a prior conviction for aggravated robbery in Tennessee in 1996, for which he was sentenced to 12 years in prison, according to court records. He also was arrested in Georgia in 2005 for investigation of murder, armed robbery, aggravated assault and kidnapping, "though the case was ultimately dismissed," according to an affidavit filed in the case by FBI Special Agent Beth F. Alvarez.

    The public defender listed for both didn't immediately respond to a voicemail or email seeking comment.

    In a statement released by the team, Jackson said the extramarital affair occurred when he was still working as an ESPN/ABC broadcaster. He called the affair "embarrassing" and apologized for the affair.

    Jackson said a man approached him this past April 3 at the team hotel in Memphis when Golden State was there to play the Grizzlies and demanded "a substantial sum of money" or he'd sell "personal information" to a tabloid.

    Jackson said he "regrettably" paid the man, calling it "a terrible lapse in judgment and a course of action I would not recommend to anyone." Court documents show he handed over $5,000.

    After the man emailed him two weeks later and called his wife asking for more money, Jackson said, he informed the team about the situation. The Warriors said they immediately contacted the FBI.

    Jackson didn't respond to an email from The Associated Press seeking further comment.

    In an email from Shaw dated May 3, he told Jackson he could get $185,000 "from another resource" if the coach didn't pay, according to the court documents.

    Jackson worked with the FBI to record several phone calls and monitor emails.

    In response to Shaw's second demand, according to the documents, "V1" wrote: "If I had known that was amount I would have offered to pay more the first time. I will give you $200,000 to keep this quiet. We can still work this out. I have the money. I do not want these pictures getting out."

    The court documents also note several text messages between Shaw and Adams that outline the alleged extortion attempt.

    On the afternoon of April 3 before Jackson said he handed over money, Adams sent a text message to Shaw that read: "Concentrate and (expletive) him up. He is a Fake (expletive) man of god," according to court documents.

    Jackson said he made his wife, Desiree, aware of the affair years ago and the two reconciled.

    Jackson, coming off a 23-43 record in his first season as a head coach, is also an ordained minister. He leads a non-denominational congregation with his wife in the Los Angeles suburb of Van Nuys. The couple has four children.

    "I recognize the extremely poor judgment that I used both in having an affair six years ago - including the embarrassing communication I exhibited during that time - and in attempting to deal with the extortion scheme at first by myself," Jackson said in the statement. "I made some egregious errors. I apologize for any embarrassment I may have caused my family, friends and, of course, the Warriors.

    "At that time in my life, I was not pastoring. Three years ago, my wife and I established a ministry. With deepest regret, I want to apologize to my Church Family. I was wrong. We must live Holy."

    The team also released a statement supporting Jackson but not condoning his affair.

    Jackson included a quote from the Bible in his written statement and asked for forgiveness for his actions.

    "What goes on in the dark," it reads, "will come out in the light."

    To improve school nutrition and get children to eat healthier, involve parents, teachers and school administrators, new research finds.

    Researchers from Kaiser Permanente Southern California looked at 400 students at eight elementary and middle schools who took part in a three-year study that examined the use of a public-health approach to improve nutrition.

    During the study, the researchers worked with teachers and administrators at certain schools to improve nutrition practices. For example, they replaced food and beverage classroom rewards with non-food prizes and implemented healthy catering at school events and classroom celebrations.

    For fund-raising events, they served healthy foods and beverages, awarded non-food prizes and had games such as a prize walk instead of a cake walk. The researchers noted that schools actually made more money through healthy events such as jog-a-thons than carnivals with popcorn and pizza.

    There was a 30 percent decrease in the amount of unhealthy foods and beverages consumed by students at these schools during the study, compared to a 26 percent increase at other schools.

    The amount of healthy lunches students brought from home and other outside sources also increased at the intervention schools.

    Schools are an ideal place for establishing lifelong healthy eating habits, but until now thats been easier said than done, study lead author Karen Coleman said in a Kaiser news release. She added that the study helped us understand how communities and schools could work together to get kids to eat healthier at school and help address childhood obesity.

    The study was published in the International Journal of Behavioral Nutrition and Physical Activity.

Facing weekend deadlines for action, congressional leaders have agreed to deals overhauling the nation's transportation programs without a Republican provision forcing approval of the proposed Keystone XL oil pipeline, and avoiding a doubling of interest rates for new student loans, congressional officials said Wednesday.

    The agreements underscored the pressures both parties face to avoid angry voters and embarrassing headlines in the run-up to this November's presidential and congressional elections. Letting road-building programs grind to a halt during an economic downturn would be a blow to the image of lawmakers, while Democrats and Republicans alike seemed eager to avoid enraging millions of students and their parents by boosting the costs of college loans.

    In contrast, enactment of the transportation measure would create or save 3 million jobs, said Sen. Barbara Boxer, D-Calif., chief sponsor of the Senate version of the bill. And the student loan measure would spare an estimated 7.4 million students who get subsidized Stafford loans beginning July 1 ? this Sunday ? from facing $1,000 in higher interest costs over the lives of their loans, which typically take over a decade to repay.

    Congressional leaders were planning to combine the highway and student loan measures into a single bill to reduce potential procedural obstacles and hoped for final approval this week. Lawmakers would then leave Washington for a July 4 recess.

    The two-year highway bill would prevent the government's authority to spend money on highways, bridges and transit systems from lapsing on Saturday, along with its ability to collect gasoline and diesel taxes. With both parties checkmating each other's top priorities this campaign season, Democrats and Republicans say the highway measure will be Congress' top job-creation initiative until the November elections.

    "This is the jobs bill for the 112th Congress," said House Transportation Committee Chairman John Mica, R-Fla.

    As the price for the highway agreement, lawmakers said Republicans dropped a House-approved provision requiring the government to approve the proposed Keystone pipeline, which is to move oil from western Canada to Texas' Gulf Coast.

    Pipeline approval ? which prompted a veto threat from President Barack Obama ? has been a top goal this election year for the GOP, which has pitted its claims that it would create jobs against environmental worries that it could accelerate global warming and endanger Midwestern water supplies.

    Republicans also agreed to abandon language blocking the Environmental Protection Agency from regulating the toxic ash generated by coal-fired power plants, lawmakers said. The ash is used as an ingredient in some types of cement.

    In return, House Republicans won Senate concessions that would halve the time allowed for environmental reviews for highway projects, and squeeze money for bike paths and pedestrian safety projects by forcing them to compete with other transportation projects, said congressional aides and environmental lobbyists.

    The bill would give states more flexibility in spending federal money, impose new safety regulations and expand a federal loan guarantee program to encourage private investments in transportation projects.

    Despite the measure's short-term impact, the bill delays for two years decisions about a long-term funding scheme for highway and transit programs. Gas and diesel taxes no longer cover the cost of transportation programs and are forecast to bring in less revenue as the fuel efficiency of cars and trucks increases.

    President Barack Obama spent weeks this spring touring college campuses and lambasting Republicans for not pushing a freeze on student loan rates through Congress. GOP presidential challenger Mitt Romney, looking to avoid handing Obama an issue, quickly said he favored the interest rate extension and congressional GOP leaders did too, though some Republicans think the government should not spend money to keep loan rates low.

    The student loan pact would keep today's 3.4 percent interest rates on subsidized Stafford loans from doubling for new loans approved beginning on Sunday, an automatic increase that Congress enacted five years ago to save money. If they did double, it would affect 7.4 million students expected to get the loans over the 12 months beginning July 1, adding $1,000 to the interest costs of the typical borrower over each loan's life.

    The tentative agreement was on the same package Senate Majority Leader Harry Reid, D-Nev., and Senate Minority Leader Mitch McConnell, R-Ky., had said Tuesday that they had shaken hands on, said a congressional Republican who spoke on condition of anonymity to describe the measure's status. McConnell said he expected the House to accept the agreement.

    "We're moving, I think, towards an agreement on a transportation bill that would also include a one-year fix on the student loan rate increase," Boehner told reporters early Wednesday after meeting privately with House Republicans.

    Some GOP lawmakers at that meeting said Boehner seemed to favor the emerging deal and said it received a positive reception.

    "The general sense is there's a tremendous amount of support for getting it done," said Rep. Tom Reed, R-N.Y. "That's what's going to drive it across the finish line."

    The two parties spent the last several weeks dueling over how to pay for the $6 billion cost of the student loan bill.

    Under the agreement, the government would raise $5 billion by changing how companies calculate the money they have to set aside for pensions. That change would make their contributions more consistent from year to year and in effect reduce their payments initially, lowering the tax deductions they receive for their pension contributions.

    Another $500 million would come from increasing the fees companies pay for the government to insure their pension plans, linking those fees to inflation.

    In addition, $1.2 billion would be saved by limiting federal subsidies of Stafford loans to six years for undergraduates.

    Lawmakers decided to wrap in another Senate bill to reauthorize the national flood insurance program for five years.

    The program, which provides federally backed insurance for 5.6 million households and businesses, many in flood-prone areas, has gone deeply into the red from claims after Hurricane Katrina in 2005. The Senate legislation would try to return the program to fiscal soundness through such steps as allowing premium increases and reducing subsidies for vacation homes.

    Nora Ephron's final act played out in Manhattan on June 26 where the 71-year-old writer and movie director died from pneumonia brought on by acute myeloid leukemia (AML), one of the most common types of leukemia among adults. AML is a cancer caused when abnormal cells grow inside bone marrow and interfere with the production of healthy blood cells. The marrow eventually stops working correctly, leaving a person with an increased risk of bleeding and infections.

    Ephron?best known for writing When Harry Met Sally and Sleepless in Seattle?was first diagnosed in 2006 with one of the myelodysplastic syndromes (MDS), a category of blood diseases also referred to as "preleukemia" that can progress into AML if the bone marrow continually fails to produce enough healthy platelets, red blood cells and white blood cells over time. MDS made headlines recently when ABC's Good Morning America anchor Robin Roberts announced she has been diagnosed with the disease.

    Some types of leukemia, including AML, develop as a result of exposure to certain chemicals (including herbicides and pesticides), chemotherapy drugs (such as etoposide and a class of drugs known as alkylating agents) and radiation. Typically, however, a doctor is unable to pinpoint the exact cause in individual cases.

    Although estimates vary, there are between 10,000 and 12,000 new cases of MDS in the U.S. annually. More than 80 percent of all MDS patients are older than 60. The National Cancer Institute projects that 13,780 men and women?7,350 men and 6,430 women?will be diagnosed with AML and that 10,200 men and women will die of the malady this year.

    Scientific American spoke with Bart Scott, a medical oncologist specializing in the treatment of patients with MDS, about syndrome's progression to AML, who is most at risk for this cancer and whether there are any promising treatments on the horizon. Scott is also director of hematology and hematologic malignancies at the Seattle Cancer Care Alliance and an assistant member of the Fred Hutchinson Cancer Research Center's clinical research division.

    [An edited transcript of the interview follows.]

    What is the typical trajectory for this form of cancer?
    The trajectory of myelodysplastic syndrome is highly dependent upon two factors?the bone marrow myeloblast count and the cytogenetics. [A myeloblast is an immature blood cell that will eventually develop into a type of white blood cell.] The higher the bone marrow myeloblast count is at time of diagnosis the more likely a patient is to progress to acute myeloid leukemia. In general, the more abnormal the cytogenetics [a cell's hereditary and functionalm characteristics] are the more likely a patient is to progress to AML, but there are certain abnormalities that have a good prognosis.

    Who are the typical victims of this form of leukemia?
    The median age of diagnosis of MDS is 72, it is slightly more common in men than women, and there is a correlation between incidence and age. I would say that Ephron's course does seem consistent with MDS progression to AML.

    What are the risk factors for this cancer?
    They are based on increasing age as well as certain environmental exposures like pesticides and herbicides. This is not casual contact?we are talking about prolonged, persistent exposure. The amount of exposure required to cause the disease is unknown, however.

    Given that Ephron lived most of her life in New York City, whose residents are not typically exposed to inordinately large amounts of pesticides or herbicides, what might account for her condition?
    Some researchers have tried to understand MDS in terms of clustering, although there is very little clustering data available. The idea is to understand whether MDS tends to cluster in areas of higher exposure to certain toxic agents. In 2007 Xiaomei Ma [an associate professor of epidemiology the Yale School of Public Health] and a team of researchers published a paper in Leukemia Research regarding this phenomenon. They found that cases of MDS in Connecticut were clustered near the western border. In general, clustering supports the idea that environmental exposure is contributory to the development of a disease. This is the only paper that I know of that has demonstrated clustering in MDS.

    We are doing a similar study in the state of Washington. Hanford Site is a nuclear production complex near the Tri-Cities area in southeastern Washington [encompassing Kennewick, Pasco and Richland]. We are looking to see if there is clustering in the Tri-Cities area. But these studies are difficult given the mobility of the area's population, unknown exposure time and any delay in the development of MDS after exposure.

    What is the usual treatment?
    There are three FDA approved treatments. One is azacitidine, approved by the FDA in 2004 and marketed as Vidaza. Decitabine, sold as Dacogen and approved in 2010 to treat MDS, is another option. [The FDA's oncologic drugs advisory panel in February, however, recommended against the approval of Dacogen to treat older patients with AML. The drug's maker wants FDA approval to use Dacogen in AML patients 65 years and older and who are not considered good candidates for high-dose chemotherapy as an initial treatment for AML.] The third is lenalidomide, also known as Revlimid and introduced in 2004.

    However, the only curative treatment is stem cell transplantation. Stem cells are infused just like a blood transfusion. Sources of stem cells include cord blood cells, peripheral blood mobilized stem cells or bone marrow. Success depends on the stage of disease. The one-year treatment related mortality is approximately 20 percent. If patients are transplanted in an early stage of the disease, we have a success rate of 80 percent.

    Are there any promising treatments in late-stage testing?
    Yes, there are several drugs being investigated in phase II and phase III trials. We currently have a phase III trial open with a drug called rigosertib for patients who have failed prior treatment with azacitidine or decitabine, which are known as demethylating agents. The proposed mechanism of action for these drugs is that they alter gene expression profiles in the cancer cells and increase their susceptibility to death. Rigosertib is a cell cycle inhibitor and would prevent the cancer cells from growing and induce direct damage to the cancer cells, causing their death.

    (AP) ? Retired Texas firefighter Raul Rodriguez, armed with a handgun and video camera, had claimed he was standing his ground and had no choice but to use deadly force when he fatally shot his unarmed neighbor after confronting him about a noisy party.

    A jury decided otherwise Wednesday, sentencing Rodriguez to 40 years in prison for killing the neighbor, Kelly Danaher, a 36-year-old elementary school teacher. Prosecutors said they are hopeful the punishment will stop others from settling matters with violence and trying to use Texas' version of a stand-your-ground law as a defense.

    "I think it sends a clear message that this was not a case of stand-your-ground," said prosecutor Kelli Johnson. "And I think from his behavior, his intent, the provocation ... shows that this had ... nothing to do with self-defense."

    Rodriguez, who faced up to life in prison for the 2010 killing, will be eligible for parole in 20 years. Jurors deliberated about five hours Wednesday before reaching their verdict. The same jury convicted him of murder on June 13.

    His reference to standing his ground was similar to the claim made by George Zimmerman, a neighborhood watch volunteer who is citing Florida's stand-your-ground law in his defense in the fatal February shooting of an unarmed teenager, Trayvon Martin. Rodriguez's case, however, was decided under a different kind of self-defense doctrine.

    During closing arguments earlier Wednesday in the trial's punishment phase, Johnson referenced Zimmerman but told jurors that case was different from what Rodriguez was convicted of doing.

    Danaher's family members said they wished the jury had sent a stronger message when sentencing Rodriguez.

    "Nothing will be enough," Danaher's wife, Mindy, said. "It's not going to bring Kelly back. I just want (Rodriguez) to be in there forever."

    Kelly Danaher's mother, Connie, called Rodriguez a "hateful coward."

    "Eternal fire and damnation is not enough for what you took from us," she said in court, reading from her victim statement.

    The night of the shooting, 46-year-old Rodriguez was angry about the noise coming from a birthday party at his neighbor's home in Huffman, an unincorporated area about 30 miles northeast of Houston. The retired Houston-area firefighter, who had a concealed handgun permit, went over and got into an argument with Danaher and two other men.

    In a 22-minute video he recorded that night, Rodriguez can be heard telling a police dispatcher "my life is in danger now" and "these people are going to go try and kill me." He then said, "I'm standing my ground here," and fatally shot Danaher and wounded the other two men.

    Defense attorney Neal Davis said he was disappointed with the sentence and worried others might be wrongly charged for standing their ground in situations where they fear for their lives.

    "When you're out there in a situation where you're truly in fear for your life and you're having to make a split-second decision, sometimes those decisions might not be correct. It doesn't mean you should be charged with murder," Davis said. "But I think that's going to be the trend, that people who stand their ground in public places and who lawfully have a weapon to use deadly force face a very real chance of getting charged with murder."

    But Johnson said Rodriguez was convicted and sentenced because he abused Texas law.

    "This was not a man that truly knew the law or truly took it seriously," the prosecutor said. "He used it to hide behind it. It was a shield for him, to do what he wanted to do."

    Jurors declined to comment about the sentence.

    Texas' version of a stand-your-ground law is known as the Castle Doctrine. It was revised in 2007 to expand the right to use deadly force. The new version allows people to defend themselves in their homes, workplaces or vehicles. It also says a person using force cannot provoke the attacker or be involved in criminal activity at the time. Legal experts say the expansion in general gave people wider latitude on the use of deadly force.

    Prosecutors called Rodriguez the aggressor and said he could have walked away from the confrontation safely before the shooting. Rodriguez's attorneys argued he was defending himself when one of the men lunged at him and he had less than two seconds to respond.

    At trial, prosecutors tried to show Rodriguez had a history of not getting along with Danaher and other neighbors. Rodriguez's ex-wife, neighbors and former co-workers testified during the punishment phase that he was abusive, a bad neighbor and had once shot a dog.

    Rodriguez's attorneys did not call any witnesses to the stand before the jury convicted him. But they presented more than a dozen witnesses, including his wife and sons, during the punishment phase. One son said Rodriguez shot the dog because it was threatening his family.

    Relatives testified he was not abusive, always stressed gun safety and was not cavalier with his weapons.

    Defense attorney William Stradley said Rodriguez "understands that what he's done is difficult and obviously if he could go back and change it he would."
